It has always been my understanding that something similar to this is what orthodontists used to do, before.it became fashionable to get braces off the moment teeth were straight.

Not for a major length of time, but on a sort of "your teeth are straight, so we'll remove your braces next time - 6 weeks later or whenever' so they had a period of being held in place without much pressure from the braces - especially knowing that few people are 100% diligent with retainer wear.

Certainly I recall a number of my friends being told something along those lines.

My guess is that the primary consideration would be oral hygiene.  If you are scrupulous about keeping everything clean, my guess would be that fixed appliances might even be preferable as Essix retainers wouldn't allow circulation of saliva.  That might even be true on the lingual surfaces with Hawley retainers.  You'll probably have to earn the right to ask for the extension.

Hallo wieder Mal.
Am Mittwoch war der große Tag. Ich bekam die Spange raus. Zuerst hat der Doc den aktuellen Zustand kontrolliert und dann die gesamte Patientenakte von vorne bis hinten nochmal genau durchgesehen. Jeden einzelnen Punkt hat er nochmals genau kontrolliert. Dann gab er das OK zum Entfernen der Spange. Ich hab gleich darum gebeten, die Spange behalten zu können. Das war überhaupt kein Problem.
Zuerst wurden die Brackets oben mit einer Zange "abgeknipst". Begonnen wurde bei der Mitte und es ging nach rechts weiter bis zum 7er. Dann folgte die linke Seite, wieder mit der Mitte beginnend.
Danach wurden die unteren Brackets auf dieselbe Weise entfernt. Hierauf folgte der unangenehmste Teil der Behandlung: das Entfernen des Klebers. Hierfür wird jeder Zahn mit dem Bohrer abgeschliffen. An zwei Zähnen war ich etwas schmerzempfindlich, aber alles geht vorbei.
Nach dem Entfernen wurde mir das Ergebnis mit einem Handspiegel gezeigt und kontrolliert, ob alle Kleberreste wirklich weg sind. Gleich danach folgte die Dokumentation mit dem Fotoapparat. Hierfür wurden jeweils 2 Fotos von vorne und von der rechten Seite gemacht - einmal mit offenem und einmal mit geschlossenem Mund. Auch zwei Röntgenbilder wurden angefertigt.
Hierauf folgte die Abnahme von Abdrücken. Im oberen Kiefer wurden zwei Abdrücke gemacht, im unteren einer. Ein Abdruck wird zu Dokuzwecken archiviert, der zweite vom OK ist für das Labor zum Anfertigen des Retainers, der in Form einer Schiene realisiert wurde.
Nach etwas Wartezeit - der Doc hatte gerade ein Erstgespräch mit zwei Jugendlichen - wurde der Retainer für den UK eingesetzt. Der Abdruck war hierfür war ja schon beim letzten Termin gemacht und der Draht in der Zwischenzeit angepasst worden. Dieser wurde auf die Innenseite der Zähne von einem 3er zum anderen 3er geklebt. Der Kleber ist gerade so dick, dass man mit der Zunge den Draht nicht wesentlich spürt.
Nach kurzer Einschulung über die zukünftige Zahnpflege mit Superfloss-Zahnseide und schriftlicher Aufklärung wurde ich für eine Stunde später zum Abholen der Schiene wiederbestellt.
Nach der Pause bekam ich die Zahnschiene für den OK, welcher gleich eingesetzt und auf Passform geprüft wurde. Auch der Doc versicherte sich über den guten Sitz. Das Einsetzen und Entfernen soll jedes Mal von hinten beginnend stattfinden, damit die Schiene nicht bricht. Auf der linken Seite ist es jedoch etwas schwierig, sie wieder zu entfernen. Zur Aufbewahrung bekam ich noch eine kleine Box aus Kunststoff, des Weiteren wurde noch ein Kontrolltermin in 4 Wochen ausgemacht.

Meine Erfahrung von nun 3 Tagen Zahnschiene: Es ist immer noch etwas schwierig, die Schiene morgens zu entfernen, teilweise schmerzen die Zähne für einen kurzen Moment leicht. Beim Einsetzen spüre ich einen geringen Druck. Dies kommt wohl davon, dass der Abdruck genommen wurde, nachdem ich die Gummis bis zum Termin getragen habe.

Jetzt ist es manchmal ein seltsames Gefühl, dass keine Spange da ist. Manchmal merke ich das auch beim Essen, dass ich immer noch vorsichtig bin.
Was die Außenwelt betrifft: Es ist bisher nur einer Person in einem Zoom-Meeting aufgefallen, dass meine Spange weg ist, bzw. wurde ich darauf angesprochen. Auch Leute, die mich öfter sehen und mich damals wegen der Spange fragten, haben es nicht unbedingt bemerkt.

Ich halte euch weiter auf dem Laufenden, aber denke, dass es nicht viele Neuigkeiten geben wird. Die Retainer werde ich jetzt ca. 1 Jahr tragen müssen.

Yes that works. My ortho asked me if I would like to wear it for 3 months longer. Just say that you don't deal well with loose things

Don't have the answer but it's one of my absolute dream that fixed braces could be used as a retainer after a treatment, temporary or permanent, to make the treatment last more and more.

Just wondering if you've been told that you'll have to wear Essix retainer 24/7 for two months after the braces come off, could you ask your ortho to retain your teeth for two months using braces instead of a retainer?

Thanks for confirming. I think she'll go with clear and see how badly they stain and then go for black if they're bad.

I did look at self-ligating but so few dentists in our area do them.

Of all the people with metal braces I've seen that don't have bright ligatures, I can't recall seeing any that I thought looked badly stained. Hopefully it's quite subtle.

The first picture on that webpage has clear ligatures, the second has light pink powerchains, and the third has no ligatures (they are Damon braces, which are a kind of self-ligating bracket).

If your wife really doesn't want colors and doesn't want to worry about staining, find an ortho who uses self-ligating brackets. Though it's possible she'll have powerchains at some point even with self-ligating brackets, and powerchains will have to be some sort of color (even if it's silver or clear).

Silver is the color that will most look like no color. Blues and dark colors will make the teeth look whiter.

They do get changed out, but it'll be every 4 to 8 weeks. So it's not like you can have red and green just for the week of Christmas, for example. I have braces right now and my appointments are usually 8 weeks apart. That makes picking a color harder because if you go with something seasonal, you have to want to have it for two months, basically.

They're clear ligatures on that image. Re cleaning the stains, it depends on what's stained it I guess - sometimes I she'd have a fluorescent yellow/green powerchain or ligs after a curry or something, that would usually fade over a week

Though from close up photos I look over the years, clear elastomerics always end up a dirty yellow, whatever you eat. Black is guaranteed no stain, white teeth. My missus wasn't into the idea of colours, but ended up enjoying switching her smile up

Thanks for that. So you're saying most people with metal braces will have elastomeric ligatures most, if not all, of the time?

We do eat a lot of foods that would stain so I guess black is best then if you want something neutral.

When I do an image search of metal braces I get what appears to me to be either braces with really saturated ligatures, like your wife had, or, the braces just look all metal. E.g. look at the images here: https://www.manchesterorthodontics.co.uk/products/metal-braces

If you stain the clear ones (or any of them) is it possible to clean the stains off? If I understand correctly they get changed quite frequently so shouldn't be too much of an issue regardless.

You can't choose metal or elastomeric ligatures, they're dependent on the treatment

Metal ligatures are used to 'lock' brackets to the wire, the prevent movement - typically towards the end of treatment. Wire ligatures are also used when there's a risk of an elastomeric ligatures pulling off the bracket, for example if a tooth is behind the arch and being pulled forward

Silver is generally a safe choice of ligature, though curry will stain it an unpleasant muddy green for a week or two. Clear turn yellow from pretty much anything bar the blandest foods, ditto white. My wife (late 30s)got many colours towards the end of treatment - her favourite was teal/turquoise. Black was good too, it makes the teeth look super white and can't stain

If you get traditional metal braces, what are the ligatures usually made of? I've read they can be metal or elastic. Is this purely cosmetic? I.e. the rubber ones are for only if you want colours. My partner is way too old for colours and we just want all metal.

Will we get a choice of metal or rubber or is it down to the treatment plan? If rubber, what is the best to go for to blend in with the metal? From what little I know you can get white, clear, silver/grey and black, all neutral. Would we go for silver? I had read that the clear ones can stain.

Ah, so those triangular brackets are fast braces. I did wonder, it's not often you see them. Tbh, I don't find them at all attractive compared to traditional metal or even selfligating or ceramic

My wife had GAC in-ovation C brackets for 6 years or so (missed appointments/ortho retiring/moving out of a city/covid), when she started at a new ortho, he preferred to work with traditional metal so changed out all her brackets for metal for the last couple of years

Thanks for the replies. I think my situation is different to Mike's as she has moderate-to-severe crowding and a cross bite that causes pain so she will really benefit from treatment.

Thanks for your detailed post, Sparky. I totally agree about fastbraces. I can only find two practitioners of Damon Braces in our entire city and neither are near to us. I have to say, Damon Braces did not ring the same alarm bells with me as fastbraces (though up until a month or so ago I knew almost nothing about braces). I thought the self-ligating nature of them just meant you had constant pressure rather than having to rely on tightenings so it spend things up a bit. It's irrelevant anyway as they're not an option for us.

We have talked at length about it now and want it done properly, not just cosmetic. Of all the reading I've done it looks like fixed metal is the most reliable, predictable option — and to fix her problems it's worth spending time on it.

I'm not sure if it's the same everywhere but in the UK pretty much every dentist website I've looked at has a huge push on anything that isn't traditional braces and is, in many cases, cosmetic only. I've read so many poor experiences of fastbraces, people who went with Invisalign and regretted it. Conversely, traditional braces seem to always do the job and most people seem fine wearing them after the first month. I think it's a shame we have a culture that is so superficial.

Kurzes Update von heute: Mir ist vorige Woche aufgefallen, dass die Lücke links, wo früher der 4er war, wieder größer geworden ist. Das merkte ich beim Verwenden der Zahnseide. Deswegen hatte ich einen Termin für heute ausgemacht.
Der Doc hat es kurz angeschaut und es für gut befunden, dass ich den Termin ausgemacht habe. Anscheinend ist der Kieferknochen noch nicht ganz so stabil, wie er sein könnte. Deswegen verschiebt sich dort leichter etwas. Aus diesem Grund ist die Verblockung oben wieder weggekommen und ich habe erneut eine Kette von 7 auf 7 bekommen.
Auf die Frage, wie es mit dem Abnahmetermin in ca. 2 Wochen steht, meinte er, dieser bleibt. Die Gummis soll ich weiter verwenden, obwohl ich mir noch nicht sicher bin, ob ich heute nicht aussetze, da ich so schon leichte Schmerzen habe. Ich konnte gerade nicht mal ein dünnes Stück Schoko ohne Schmerzen essen.
Alternativ könnte ich auch einen weicheren Gummi einsetzen. Ich werde das in den nächsten Minuten entscheiden.
Morgen sieht alles sicher wieder ganz anders aus. Also abwarten und Tee trinken...
