The pins are set into the bone as anchorage. The elastics are pulling the molars. I've seen something like this discussed as an alternative to headgear--using the bony structure inside the mouth as anchorage instead of the neck basically.

When it is used in place of headgear the pins are placed in other locations.

Isn't that like the bands on the side of the braces? Not the ones around the brackets but the two on the side of the mouth.

By the look of it, the patients rear molars are rotated to far outwards, and this is pulling them back in again.
I'm wondering how else that could (as effectively) be done? Some form of TPA?

Ouch! I don't see much done to molars but those needed it.
