It looks like he's wearing regular brackets along with a retainer and headgear. Does anyone know what appliance(s) I'm looking at? I've never seen anything quite like this before.

Its a Aktivator whit Headgear and fixed Braces.

I remember this photo very well. There was a video on youtube where the boy showed this device and talked while wearing it. I can no longer find the video.
Try to search for it on youtube by typing the search in different languages

It's a kind of activator with spurs to push the upper teeth back instead of a bow. You can find similar ones on (German) Ebay occasionally. In this case, headgear added. Still, it's a nice one!
