Most people don´t notice anything.
I had to wear for several years hawley expanders and even a Bionator.
The expanders I had to wear 22 hours a day, so it was normal to go out with them in my mouth. The only challenge was that the gap of the expansion screw got wider and wider within the treatment so i got after a while a nice retainer lisp. Then the people looked up noticing the wire and the two hooks in front of the teeth but nobody asked me about my devices in my mouth - everybody knows for what retainers are made for.  After my treatment was over, I suddenly missed my retainers so I´ve ordered the same type again to have my retainers back and I´m still wearing them, but of course not further activating them anymore, so the speech is clear and nearly nobody notices them.

I’ll add my voice to the chorus – people literally don’t give a crap.  As that old song said: the Martians could land in the car park and no one would care. 

Many years ago, I did an experiment with a model I did a few photoshoots for. She had braces at the time, and I wanted to add to my ‘Braces are Beautiful’ series which I had on DeviantArt at the time.  (long since closed down, don’t even bother asking for pics).  I convinced her ortho to give her a headgear for the sole purpose of a shoot. 

After the shoot I dared her to go for drinks with me with the device still attached.  We went to a very upmarket and very busy coffee shop, and she entered with her headgear on.  No one friggin’ noticed.  Even the model commented on how little impact it had on the public. 

And my own public headgear experiences – there were not many, but I had them – were ultimately very underwhelming, a much bigger deal in my own mind than it ever was in reality.

Nobody gives a crap.  If nobody gives a shit about a very obvious headgear, then even fewer people will care about standard issue railway tracks. 

Ask me if this was true in the 80s when people were not glued tot heir phones all day and I’ll give you a different answer.

Years ago while having Invisalign I had a few stubborn teeth and finished off treatment in braces. I was terrified to be honest at how they’d look and how I’d be perceived. I had them for about 3 months and aside from family and friends. I had one person the entire time who mentioned about them and it was only because she was currently in braces as well. I figured they’d be a lot more of a conversation starter but I also see the flip side about bringing up a conversation that way and how it might be perceived as strange

I've had mine for a couple of years, and besides showing my best friend from high school who was excited for me to finally ge them, since she was well aware of my kink for them, not many other people, if any at all, ever really reacted to them. My family would notice and say things like "oh I'm so sorry that's rough buddy". At one of my more memorable jobs, one of my coworkers commented "woah I didn't know you had braces!" After I'd worked there for a year and had them the entire time I'd worked there. This told me I needed to smile a whole lot more. I think mostly itll be people I haven't seen since high school seeing me and chatting for a bit, there's about a 50/50 theyll comment on them, but most try to be polite and not bring it up, One of the many reasons I want to be prescribed an extra oral appliance of any kind. Anything to draw attention to them lol. Since I got them right outa high school, there wasnt really a point I was scared to be getting them, i was just so excited to finally have them!

Did you take photos out in the coffee shop? I vaguely remember an article with photos where someone had done literally that - worn headgear and braces to a cafe or restaurant or something. Wonder if it was your model...?

It really depends of the situation, if I'm just walking in the street with my (fake) braces and headgear, post of the time people just take a quick look and then try not to stare, if I'm having a conversation with somebody, the person will probably ask me what is in my mouth and then I can engage a conversation about my orthodontic treatment.

When i got my braces put on people noticed but no one really cared. Most people were like good for you. But no real embarrassing moments just the ones in my head. Im about half way done with them.
