Birthday present

Chapter 1

It had been months since they had been talking about it. It started with as it often went. "What if ...." And then it was about what you would study if you could start again. Or where you would live.
This time it was about appearance. What you would like to change about how you looked.
He was actually very satisfied with his appearance. At the age of 49 he still looked good. And the most important: he was very satisfied with it.
As long as he knew her, his wife had been uncertain about what she looked like. No matter how much he praised her, she always had something she didn't like about herself.
"But you never have that, everything is perfect for you", she had said.
In order not to appear too self-righteous and to reinforce his position that nobody was perfect, he had said that he also saw some improvements in himself.
"What then?"
He had stuttered and murmured that he really did not find himself so beautiful or handsome. "But if you could change something about yourself, what would it be?"
"My teeth," he said.
His teeth were indeed not perfectly aligned and somewhat forward. But he had never been bothered by it.
"But why don't you have something done about it?", His wife had asked. "Are you sometimes too shy to walk around with braces?"
"I really wouldn't care."
"Not even if, for example, you should have to wear a headgear?"
"No not at all."
"And if the treatment would take a very long time?"
"I wouldn't care," he replied very firmly, knowing that it would not happen that he would undergo orthodontic treatment. He had a nice salary, but he could not afford such treatment. And even if he could, he would never start a treatment, for sure.

Chapter 2

Where the hell was his mouth guard? He had been playing field hockey for years and had such a protector made long ago. But he couldn't find the thing anywhere.
"Left it at the club", his wife had said. But he could not imagine that: he was always very precise about his things.
His wife had offered to find an address where he could have a new mouth guard made. And so it happened that he was in the waiting room at an orthodontist on Monday morning. Next to him was a 30-year-old woman with a highpull headgear, opposite him a 40-year-old man with a lot of metal in his mouth.
He thought how nice it was that it had all been spared him. And that in three weeks he and his friends, family and colleagues could celebrate his 50th birthday without significant health problems. And in complete satisfaction with himself and his life.
"Mr. Paul Smith?" He heard the receptionist calling in his direction. "You can walk with me."
She led him to a large room with perhaps ten dental chairs. Every seat, except 1, was occupied. Assistants were busy installing fixed brackets. In the corner he saw a child that had just been fitted with a headgear crying. "He cannot get out, he is stuck", he heard the assistant who was busy with the child say.
He himself was allowed to sit in the free chair. "Then we will immediately make the impressions" said the assistant who was assigned to him.
Making the impressions was of little use, he remembered from years ago. It was also ready in no time.
The orthodontist just introduced himself. It was a woman in her early 30s. "Welcome to our practice. Have you ever been to an orthodontist before?"
His previous mouth guard was made by a dentist. So this was the first orthodontist practice he saw inside.
"May I have a look at your teeth?"
Before he knew it she was looking at his teeth from all sides.
"Have you ever considered starting brace treatment?"
He said he and his wife had talked about that. But that they didn't have the money for that.
"Understandable. Although the costs are often used by many people as an excuse because they would be embarrassed if they had to walk around with braces later in life."
"I wouldn't care."
"Really? Not even if that is more complicated equipment than the most common fixed bracket?"
"I happened to talk about it with my wife a while back. Even such a headgear would be fine. And how long it would last would not matter to me either. But unfortunately it is out of the question."
That 'unfortunately' was naturally lied: he wanted to look tough.

And it would never come to an orthodontic treatment, so he could safely say that.
"I'm sorry for you," the orthodontist continued, continuing to look at his teeth.
"Well, I'll let the assistant make some x-rays and then your mouthguard will be ready in two weeks. You can make an appointment at the reception to fit it."
He thought the orthodontist was a nice woman. And he thinks she did a thorough job, even having photos made of his teeth. He was convinced that it became a mouthguard that was a little nicer than his previous one.
After the photos were taken, the receptionist planned a new appointment. Friday in two weeks, a week before his 50th birthday, he could come and fit the thing.

Chapter 3

The orthodontic practice had called, his wife told him, a week before he had an appointment to fit his mouth guard. Whether the appointment could be made a week later, on his 50th birthday. "Early in the morning at ten o'clock. I said it was okay. Afterwards we can go into town for a while."
The practice was in the center of the city. At the end of the afternoon he would have a big party, all his colleagues were also invited. He was really looking forward to it. And oh well, just past the ortho in the morning didn't matter much. Would never last longer than fifteen minutes after all.
The day it was his birthday started happily. His wife had prepared a nice and large breakfast and told him that he would get his birthday present when they were in the center. She was happy and excited about the surprise she had in store for him. "I think you're going to be very happy with it."
They arrived at the orthodontist in good time. In the waiting room he had talked to a man of about forty with a scaffold around his head. "A facemask," he had said. "Have had the braces for a year and a half but still find it terrible every day."
Paul could imagine. It was a striking white scaffold, with a band for the forehead and a white bowl on the chin. And then there were bars between with elastics that went into the mouth.
He thought how happy he was that he didn't get braces and only came for a mouth guard. His wife lovingly pinched him. "I'm sure it's your turn soon."
She had hardly said if his name was called. "Come in please."

Chapter 4

The assistant showed him the chair on which he had to take a seat. His wife was allowed to sit on a stool.
"We will now first thoroughly clean your teeth", assistant said as she lowered his chair back.
He actually wanted to say that he only came to fit the mouth guard, but changed his mind
After all, dental cleaning was never wrong. Probably a kind of service.
He glanced around for a moment. There was a patient in every chair. Next to him lay a woman with a lip spreader in her mouth who was given a fixed brace. Opposite lay a man who was looking at his new braces with a mirror in his hand. As far as he could see the man had quite a bit of iron work in his mouth. The man also slipped quite a bit.
The dental cleaning was thorough, but went quite quickly. "Rinse for a moment, Mr. Smith."

Chapter 5
His wife smiled at him. He gave her a sweet look back. He was so lucky with his wife. He was looking forward to the rest of the day. Soon they would first have lunch together and buy a new shirt for the party. And he would also go to the hairdresser. "Open your mouth wide", he heard the assistant say. He obeyed obediently: the sooner he was finished here, the better it was for him. It all took him far too long actually.
The assistant placed something in his mouth, pulling the lips away from the teeth. His tongue was also pushed back. He wanted to ask what this was for, but he couldn't make himself understood. "We are now placing an eye mask so that you can relax a bit more."
He saw the assistant put a black mask in front of his eyes, so that he could no longer see anything. Strange, he thought. But given the fact that they had already had their teeth cleaned, it must be part of their thorough way of working, he thought. He calmly waited for what would come. Dreamed a bit away.
Every now and then he felt something press against his teeth. And something seemed to be scraped from his teeth. Further dental cleaning? He thought it a bit exaggerated, but decided to surrender to it. Nothing could ruin this day, after all, and soon he would be able to go out with his dear wife.
"Bite your teeth together", he heard the assistant ask. "And again," she repeated several times. He felt some pressure between the teeth of his lower teeth. What the hell were they doing?

Chapter 6
"Oh, you are very far already," he hears a voice beside him. He recognizes the voice of the orthodontist who compliments the assistant. "Well done."
He feels that someone is touching the face mask and pulling it away. He looks straight into the radiant face of the orthodontist, who holds out her hand to him. "Happy birthday, Mr. Smith."
He wants to say thanks again, but that is impossible. "I will just take the mouthpiece away, it is no longer necessary", the orthodontist laughs. She takes the spreader out of his mouth. Immediately he feels that there is something on his teeth. And under his tongue. The orthodontist is seeing6 that he goes along with his tongue. "Your birthday gift", she laughs. And she begins to say that his wife had called the practice a while back with the idea that she wanted to surprise him for his birthday. That he had said he really wanted braces but that they had no money for it.
The orthodontist had invited his wife to come and talk. Coincidentally, she had a research program underway that examines how adults can be treated without the need for surgical surgery. "It takes longer and because we sometimes use special equipment is a lot more expensive." But participants in the study only had to pay 10 percent of the costs. "And we were still looking for candidates."
By supposedly losing his mouth protector, it would have been easy to get him into orthodontic practice. "And there you confirmed what your wife said: that you were highly motivated, the ideal candidate." He glanced at his wife. The orthodontist gave him a mirror. Large silver-colored brackets were on his teeth. The two molars in the upper jaw were still free on both sides, just like 1 molar on both sides in the lower jaw. However, an expander was placed there. "You get something like that in the upper jaw," the orthodontist said when she saw him carefully inspecting the expander. "But first sign the forms."
She put a contract on his lap. That said about the 90 percent discount, but that he was obliged to cooperate well and finish the treatment.
He felt panic. What could he do? Don't draw? But after all that was said, it was difficult for him to return. But at the same time he did not want braces at all.
"What a nice surprise, isn't it," his wife smiled at him. That too. Several thoughts flashed through his mind. Doubts. He felt once again what was in his mouth. What this really bad? Another expander for the upper teeth was added ... couldn't it be that bad? He signed the contract and handed it to the orthodontist.
