A routine checkup
« on: 20. November 2020, 21:51:28 PM »
I had always prided myself in my straight white teeth. I had braces in 8th grade for a year with the standard rubber bands and since then wore my retainer every night. I was 21 now and figured that a few nights without my retainer wouldn’t harm anything, after all it had been 7 years. For the past year I had really only worn my retainer once a month when I thought about it. I was back home for winter break and had a schedule for a dentist checkup. The dentist I usually go to had recently closed, but there was a new one in my area that took my insurance, so I figured I would just go there. Dentists were all basically the same after all, right?

I woke up the morning of my dentist appointment dreading it. I hated having people put their hands in my mouth, and the toothpaste they used is so cakey and always tasted gross. I also knew exactly how the appointment would go. They would tell me that my teeth are so straight and white and that they couldn’t find any issues with them. I had never even had a cavity.

The new dentist really didn’t look any different than my old one. The only real difference was my old one had private rooms, but in this one there was just a line of chairs that everyone sat in. I didn’t love the lack of privacy, but I would be in and out fast so no big deal.

The dental hygienist came and began the painstaking process of flossing and brushing. I tried not to gag when I tasted that disgusting toothpaste. I guess every dentist just used that kind.

“When is the last time you got x rays done?” the hygienist asked.

“I’m not sure probably not for a couple of years. Why, do I need them?” I asked.

“Dr. Brian always likes x rays for new patients. Is that okay with you,” she asked.

“Yeah that’s totally fine!”

My old dentist had never taken x rays before, but it’s not like they’re painful so why not?

The x rays were normal, just annoying like everything else. I pulled out my phone while I waited for Dr. Brian to come finish the checkup. It took a good ten minutes before he came over and I wondered what took him so long.

“Hi Gracie, I just finished taking a look at your x rays, can you open wide for me?” Dr. Brian asked walking over.

“Sure!” I said awkwardly opening my mouth.

“So this confirms what I saw on your X-ray. Have you been wearing your retainer?” he asked.

“I haven’t been wearing it as much lately, why?”

“So I’ll show you this in the mirror, your upper teeth have started shifting inward making your bite more narrow, and your front two teeth have shifted so one of them is rotated,” he said handing me a mirror. “Do you see what I mean?”

I studied my smile in the mirror. I hadn’t even noticed that my front teeth had gotten a little crooked.

“I see what you mean. Is it a big issue?” I asked.

“It might look cosmetic but it will end up giving you jaw problems later in life if it doesn’t get fixed,” he replied.

“Fixed like braces?” I asked.

“Something like that. I can refer you to an orthodontist if you’d like.”

“Hmm I’ll set up an appointment and see what he says while I’m home,” I said knowing that I most likely wouldn’t do anything about it. I was too old to get braces after all.

“Alright when you go to check out they will help you set up and appointment and I will send your X-rays over to them too,” Dr. Brian said.

“That’d be great! Thanks,” I told him getting up to leave.

The receptionist checked me out and told me that the orthodontist, Dr. White, had an appointment available in the morning. It seemed too soon but I agreed to go in the morning. This definitely did not go how I planned.

When I walked into the orthodontists office the next morning I was immediately hit by the smell. I always hated the smell of the orthodontist like rubber and cleanser. I gave the receptionist my name and she directed my to the waiting room. The majority of the people waiting were younger, but I did see a few people who looked like my parents age waiting.

“Gracie! You’re up,” a young woman who I assumed was Dr. White’s assistant called.

I smiled and followed her to a chair. I did not understand this new open floor concept I liked having privacy!

“Hi Gracie, I’m Dr. White,” said a clean cut older man out stretching his hand for me to shake it.

“Nice to meet you.”

“Let me take a look at those teeth of yours. Open, bite down for me. Hmm let me see you swallow,” he said brown furrowing a little. “I’m going to be honest with you, this is not what we like to see.”

“Oh really?” I asked a little nervous now.

“It looks like your jaw changed some since you last had braces and there were some habits that were never corrected. Did you have any appliances?”

“No, just braces and two rubber bands.”

“So, to avoid issues in the future we need to start treatment as soon as possible. If you sign some routine paperwork for me today we could get you started as soon as this week,” he said.

This was all going way faster than I expected.

“I don’t know if braces are in my price range right now,” I said trying to think of an easy way out.

“I think we could get everything done within your parents insurance on the plan that I was thinking.”

“You really think it’s that serious?” I asked.

“Unfortunately yes,” he said. “Let me get you the paperwork and I’ll give you a minute to look over it and think it through before signing.”

He handed me a stack of documents. I briefly scanned them but I had a bad habit of never fully reading before signing. I looked in my phone camera, my teeth really could be better. The first time wasn’t horrible and I might as well get it over with now. I sighed and picked up the pen and signed.

“I’m ready Dr. White, I signed!” I said.

“Let’s get that treatment started,” he smiled.

Getting the treatment started meant taking several molds of my mouth with the disgusting trays or goop that I had always thought I would never have to put in my mouth again.

“I’m going to send your molds in and we should get everything we need back by next week!” Dr. White said.

“Am I good to go then?” I asked him wanting to get out of the office and contemplate what I just agreed to.

“I’m going to add a few of these spacers in between your teeth to make the banding process go easier for you.”

I hated the sound of that. Banding process sounded so intense. I opened wide again as he stuck little blue bands between each of my three back molars so twelve in total, which seemed like a lot more than I remembered the first time around. I hoped it was because treatment was more efficient now or soemthing.

“You are good to go! Just check out at the front desk and schedule an appointment for next Monday.”

“Great, thanks,” I said getting up and walking to the front desk.

“So next Monday at 12?” the receptionist asked. “It will take around 4 hours so you will need your day blocked off.”

“Four hours?” I asked. “Is that normal?”

That seemed like a ridiculously long time to put some braces on.

“For your treatment plan, yes,” the receptionist replied.

“Then yes that works I guess.”

Why did people keep saying this treatment plan. Isn’t everyone’s braces somewhat the same?

“Alright we will see you then!”

I came back the next Monday nervous. I had stared at my smile for so long that morning and almost called to back out several times. When I arrived at the office I noticed someone around my age there with literal headgear coming out of their mouth. I fought the urge to stare in horror.

“Gracie you’re up!” a cheerful assistant called.

She looked at her chart as she led me to my chair. “Oh wow I see you chose the compliance plan,” she said.

“What do you mean?” I asked sitting down.

“Oh I’m sure Dr. White will explain it!”

I sat down in the chair looking at my phone while I waited.

“Hi Gracie how are you today?” Dr. White said smiling.

“As good as I can be,” I said laughing.

“Well let’s get you started!”

“Oh what did your assistant mean by the compliance plan?” I asked.

“I’ll explain once everything is in,” he said vaguely.

That response coupled with the word everything made me nervous. His assistant came over and put the lip spreader in and my mouth was pried wide open now. I could see out of the corner of my eye a tray with what looked like more than just braces on it. The next four hours was a painful process coupled by the fear of when would Dr. White stop reaching over to that tray.

“Alright we are done,” Dr. White finally said. “Lucy can you take the spreaders out and explain the process to Gracie, while I check on another patient. I’ll pop back over in a little to see if there’s any questions.”

Lucy took the lip spreader out and the dryness my mouth felt this whole time was immediately replaced by saliva. I felt a little panicky as I realized my tongue had no place to go, but I forced myself to calm down as I was sure there was just more instruments in my mouth. Lucy didn’t move to take anything else out though.

“Caaan you thake the retht out pleathe?” I asked horrified by the lisp and drool coming out of my mouth.

“There isn’t anything to take out. Those are your appliances!” she said smiling as my eyes widened in horror.

“Why am I lithping? I thought I wath jutht getting brathes?” I asked horrified that this was going to be how I sounded.

“So, we have started you with a few different appliances today, and for your treatment plan just braces would not be enough. What is probably causing your lisp is the palatal expanders. We put one in your upper jaw and one in your lower. These are going to help widen your bite, and you really should have had these when you were younger. Another appliance that we were shocked that you’ve never had is w tongue crib. Your tongue does not rest in the right way which is pushing your front two teeth out.”

I could feel what she was talking about in my mouth as my tongue could not only not touch the roof of my mouth from the expander but also couldn’t really move forward in my mouth either.

“The expander alone would cause a slight lisp, but with the expanders your lisp will probably be pretty severe for the rest of your treatment. We only added one other appliance today as we didn’t want to overwhelm you but this will start to treat your overbite. It’s called a herbst.”

“I can’t jutht uthe rubber bands like I did before?” I asked not loving the discomfort of the metal rubbing my cheeks.

“The treatment plan you chose maximizes the use of appliances, so rubber bands weren’t really something you were qualified for. Ah, here’s Dr. White you can ask him any other questions that you have.”

“How’s that mouth feeling?” Dr White asked.

“Well I think I want to switch treatment planth.”

“Oh really? Why’s that?” He asked sounding surprised despite the fact that it seemed obvious.

“Thith ith a little too intenthe for me. I think I’d prefer normal brathes without applianthes.”

“Well, the plan you chose is a little more complicated, and unfortunately since you wanted to pay a lower price is comes with the catch of we do it the way that gets the best results. The other plans are tailored for comfort, but I really hate not being able to do it the way I like to, so patients like you have the option to pay less to have it done my way, but you always have the option of switching your plan.”

“Yeth how do we do that?”

“So, you would have to break the contract you signed earlier which would cost you roughly $10,000.”

I thought my eyes were going to pop out of my head when he said that.

“I gueth I’ll keep them then,” I lisped as there was no way I could afford $10,000.

“Okay sounds good! Go check out and make an appointment with us two weeks from now. I want to make sure all the appliances are doing their job.”

I got up from the chair and checked out at the front desk. I had to repeat myself three times before the receptionist could figure out what I was saying. After scheduling the appointment I stopped in the bathroom realizing that I had never even seen what I looked like.

It was honestly worse than I could have imagined. Even closed you could tell there was stuff in my mouth and my mouth was gleaming with metal when I smiled. I needed to do something about this. I decided I would ask my parents about loaning me money when I got home, but deep down I knew they’d probably say no, especially since I was the one who agreed to this.

The next two weeks were torture. As soon as I got home I immediately begged my parents to lend me the money to get out of the contract. It took them an embarrassing amount of time to understand what I was saying through the lisping and spit, but once they understood they told me that I would have to deal with this on my own. My mom seemed to even agree with the orthodontist that this was for the best.
“An orthodontist knows best,” she told me.

So the two weeks leading up to my next appointment were a grueling process of learning how to speak and eat again. Besides causing a horrible lisp, the tongue crib seemed to hang too low and made using utensils almost impossible. The only thing getting me through these weeks was the hope that I could convince Dr. White to switch my plans or at least get rid of one of my appliances.

When my appointment day finally came I had convinced myself that Dr. White would agree to remove the appliances. I felt much more confident walking in to the appointment.

“Hi, ith Grathie here to thee Dr. White,” I listed to the receptionist.

“Alright Gracie, I will let him know you are here!”

I sat down in the waiting room with yet again another person wearing headgear. This time it was a boy my age and his mouth looked uncomfortably full to the point that he could barely close it. I was curious what was in his mouth that caused his jaw to be so rigid and his cheeks so full. They looked worse than mine with my herbst!

My staring was a bit too obvious as he caught my eye.

“I’m Greg,” he mumbled with a half wave. “Ith called an activator.”

He pointed at his mouth as he said this.

“Thorry I didn’t mean to thare. I’m Grathie,” I replied.

“Are you new here?”

“I have only had one appointment tho far.”

“I thought tho,” he replied and I noticed he had a very heavy lisp.

“Whyth that?”

“You’re headgear free.”

“Greg,” called the assistant. “Dr. White is ready for you.”

I watched Greg leave horrified and wondering what his words meant. Surely I would not need headgear? His case had to be much worse than mine.

I was now anxious while waiting for my turn. When the assistant finally called me I realized my hands had been balled and were a little damp. I laid down in the chair and she reclined me back.

“Hi Gracie, how are your appliances treating you?” asked Dr. White leaning over me.

“Well I wath hoping we could make thome adjuthments. The herbst bothers my cheekth and the tongue crib giveth me a bad lithp.”

“Hmm we will definitely make some adjustments and you will be happy to hear the herbst will go!”

I immediately felt much more relaxed hearing that.

“Let me change your arch wires and look at how everything is adjusting.”

Dr. White put the lip spreader in and began to examine my mouth. I felt him slide my wires out and tension as he put new ones in.

“Hmm well I was worried this would happen. Your tongue is still moving around the upper tongue crib so I will have to fix that.”

He reached over to a tray next to him.

“I will remove your herbst then adjust that tongue crib for you.”

I was happy to hear that it sounded like I was getting everything I wanted. He started installing something in my lower jaw that made me a bit nervous. When he took out the lip spreader I felt that my jaw was more free without the herbst, but now I could not close my teeth fully and I had literally nowhere to put my tongue.

“So I took out your herbst as I’m sure you can tell, but I added a lower tongue crib to ensure that your tongue cannot get around at all. I had to add some bite blocks to your back teeth to keep the cribs from getting caught together, so that will take some getting used to at first.”

I could not believe he was doing this to me.

“I caa taaalkk,” i said, my voice completely garbled.

“You will adjust, I promise,” he replied seeming unconcerned with my discomfort. “One more thing to add then we will send you on your way. I’m sure you are sick of this chair.”

What in the world could he possibly add. My eyes widened as I saw him pull out a metal u.

“Open wide.”

I reluctantly opened my mouth.

“This is your face bow it clicks into your molar bands and will be used instead of the herbst. Now lean forward.”

Dr. White pulled a navy blue strap from the drawer and slid it around my neck attaching it to the bow. I could immediately feel pressure in my jaw and like it was sliding back in my mouth. I almost groaned out loud when I saw him pull another more intricate strap out and put it around the back of my head.

“You have a lot of hair,” he said in an annoyed voice as he attached the strap. “I would recommend getting it cut as it will get in the way of the strap and could mess with the pressure.”

Great, so headgear and losing my long hair that took forever to grow.

“So this headgear had a neck and head strap for you to wear. It does not need to be worn full time. Only 18 hours a day!”

I thought I was going to pass out in the chair, this day was not going how I planned.

Dr. White’s assistant led me to a mirror in the waiting room of the office. My cheeks were bright red as she made me practice taking my headgear in and out in front of everyone. As I was taking it out she made sure to emphasize that I would not need to be taking it out much. My lips kept getting caught on the face bow and even though I was continuously slurping saliva was still somehow trickling out of the corner of my mouth. I scheduled my follow up appointment for four weeks from now like Dr. White had instructed. Greg was also leaving at the same time as me and he held the door open for me.

“Thorry to thee that you are joining the headgear club,” he said as I walked out.

“Dr. White thaid I have to wear thith 18 hours a day, but that theems impothible,” I lisped back, self conscious of how slurred my speech was.

“You’re lucky, I’m 24 hourth,” Greg replied. “Did he recommend you cut your hair too?”

“How’d you know?” I asked surprised.

“I got the same speech, I ignore him at firtht, but he threatened to make my treatment longer, tho now I’ve had a thaved head for the latht year.”

“He will have to be okay with my hair I’m not doing that!” I said. No way can Dr. White control that much of my life.

“Here ith my number if you ever need help with anything,” Greg said handing me a business card. I gave him a closed lip smile and thanked him. “Make thure to follow hith instructions it could get much worthe.”

Despite Greg’s warning, as soon as I got in my car the headgear immediately came off. No way was I wearing that except for bed.

I refused to even look at my headgear for the rest of the day. The double tongue cribs were already miserable enough I was not about to add the headgear to my problems. I quickly found out that eating was almost impossible and the drool was constant. I was dreading dinner with my parents, but I knew they’d be upset if I did not go so I decided to suck it up and let them see the newest additions to my mouth. Unsurprisingly, my mom gave me a speech on how happy I would be with the final results that was meant to be encouraging however left me feeling hopeless. I slurped my way through the meal cringing every time the fork got caught on my tongue crib.

It took a week before I worked up the courage to leave the house again when I was forced out to get groceries. I still had not brought myself to put the headgear on. I was walking down the aisle in the store looking for something soft enough to be able to chew when I heard a very garbled voice call, “Grathie!”

I turned to see Greg, the guy my age from the orthodontist office, waving at me from down the aisle. Even from a distance I could see how full his mouth was. I waved back and made my way towards him. “Hi Greg, how awre you,” I asked trying to keep my speech as clear as possible.

“I’m good, how’s your new headgear holding up?” Greg asked me. Up close I could see how much plastic was truly in his mouth. He had to speak with his teeth clamped together to keep the appliance in place.

“Honethtly I haven’t worn ith,” I replied. Greg’s face looked horrified.

“Trutht me you will regret not wearing it,” he said ominously. “I didn’t wear my activator at firtht but he thomehow will alwayth know.”

I left the store with Greg’s words ringing in my head. There’s no way Dr. White would find out I was not wearing my headgear, right? I’m sure Greg was just being paranoid. I couldn’t help but feel a little nervous for my appointment tomorrow. If Dr. White found out I can’t imagine what he’d do. It couldn’t get any worse than wearing headgear though.
